GRANT VALUE

The Duxton Dental Invisalign Grant’s are two prizes valued at up to NZD$10,000 to be used only towards orthodontic treatment to be completed at Duxton Dental.

The (up to) $10,000 prize for the Orthodontic Grant is not transferable to any other individual, it is not redeemable for cash, unused portions of the prize may not be transformed into cash, and it may only be used for orthodontic treatment received at Duxton Dental.

The prize is valid for 36 months after the date of award and the orthodontic treatment must be completed within this time frame. If the treatment has not been completed within 36 months. Duxton Dental also reserves the right to end treatment early with the consent of the patient if the treatment is not progressing according to the agreed treatment plan.

SELECTION CRITERIA

Currently enrolled students in Years 7 – 13 of the following schools may be nominated for the 2020 Scholarship:

• St Bedes College
• Villa Maria
• St Thomas’s
• Marian College
• Catholic Cathedral College

For the elimination of doubt, the student must be enrolled and attending their school on the date of the grant being awarded. If a student leaves their school before the date of the grant being awarded, they will forfeit their entry in the grant process.

Anyone may nominate a deserving student for the grant.

Applications will be received between 1 July 2021 to 31 October 2021. Applications received after this date will not be considered.

Applications can be made in two ways. Fill out and submit the online nomination form on the Duxton Dental website – see the link at the bottom of this page.

Completed forms can be scanned and emailed, or posted or dropped into Duxton Dental 22 Bartlett St, Riccarton, Christchurch, 8011

Incomplete applications will not be accepted, and Duxton Dental reserves the right to contact the person nominating to clarify details in the application.

Nominees will be considered on achievement in Arts, Sports, Academics, Cultural activities, or contributions to the Community.

In November 2021, all applications will be considered by the Orthodontic Grant Committee. Finalists will be chosen and they will be invited to attend Duxton Dental for a complimentary orthodontic consultation. The appointment will clinically assess the finalist’s orthodontic condition and confirm their candidacy for the grant. Orthodontic records maybe taken at this time including X-rays , photos and scans.

The announcement of the two grant recipients will be made after all orthodontic consultations with the finalists have been completed. This is expected to be at the end of November 2021.

The unsuccessful finalists will be presented with an oral health gift bag to thank them for their involvement in the grant.

The Grant Committee’s decision is final and no correspondence will be entered into.

ORTHODONTIC TREATMENT

The grant recipients will each be awarded a prize up to the value of $10,000 to be used towards a course of Orthodontic treatment at Duxton Dental. Treatment will only proceed with the full cooperation and informed consent of the patient, and their parents/guardians.

ORTHODONTIC GRANT WINNER OBLIGATIONS

The grant recipient consents to have their photo taken and used on the Duxton Dental Social Media platforms to congratulate them on their success.

The grant recipient will be asked to allow their clinical photos to be used for the purposes of patient education (e.g. before and after treatment).

From time to time during treatment, the grant recipient may be asked for treatment progress photos and/or their experiences to be shared with various media for educational or promotional purposes.

The grant recipient will continue or start community volunteering of at least 40 hours. This could involve volunteering time to sports coaching, School community groups, Aged care.
